Disability reports
People applying for exemption from income tax with the purpose of benefiting from the law no. 2022; they must refer to the secretariat of the Health Board with 4 photographs and photocopies of the official letter they will receive from the tax office of the Department of Public Books, the Department of Agriculture, the Department of Labor or the Department of Health, and 4 photos and photocopies of the report of the Presidency of the Health Board. The reports of the Board of Health for People with Disabilities in our hospital are organized in accordance with the rules of the Official Gazette No. 28603 dated 30 March 2013.
Reports of fraud
The Social Security Agency (SGK) prepares the reports of malpractice to determine the diagnosis and mortality rate of the person.
The labour force loss ratio (as a percentage of the total labour force loss) is determined by the SGK in the Reports on Mallulity.
Documents needed:
-The official text to be given by the Provincial Directorate of the Social Security Agency
-2 Photos from the past six months
- A photocopy of the identity card.

Two regular photographs (taken in the last six months) are required for valid reports.
Status reports
Written electronically by the relevant expert physician The reports shall be signed by the relevant expert physician in two copies by the secretary of the main branch of science. The health board report will be submitted to the secretariat. After the approval of the President of the Board of Health, a copy of the report is registered and archived, while the other copy is delivered to the patient for a signature fee.
According to the contents of the report, two photographs can be requested by the corresponding maternity branch secretary for our patients over the age of 15.
No photos are required for patients under the age of 15
Relay reports
Relay reports written electronically by the relevant expert physician shall be submitted to the health board's reporting secretariat after approval by the secretary of the main branch of science in the form of a printed document in two copies. Once approved by the President of the Board of Health, a copy of the report is archived and registered. The other copy of the report will be delivered to the patient for signature.
For a rest report of more than 10 days, patients over the age of 15 must submit two photographs to the corresponding secretary of the Anabiology Department.
Medical Supplies Reports
The reports of medical material written electronically by the relevant expert physician shall be submitted to the reporting secretariat of the health council in two copies by the secretary of the main branch of science in the form of printed documents approved by three specialist physicians.
Once approved by the President of the Board of Health, a copy of the report is archived and registered, and a second copy of the report is delivered to the patient for signature.
For the medical materials report, patients over the age of 15 must submit two photos to the secretary of the department of anabiology.
No photography is required for patients under the age of 15.
Medication reports
As of 1st of April, 2011 at Turgut Ozal Medical Center, the medication reports of our patients with SGK are prepared as e-reports.
Our patients don't need to get a printed report.
Medical reports prepared electronically by our specialist doctors can be viewed by pharmacies across our country on a computer.
Our patients can easily prescribe their medicines to different hospitals and doctors using the computer output of their pharmacy medicines.
